Indications/Uses
Management of reversible bronchospasm associated w/ obstructive airway diseases in patients who require more than a single bronchodilator.
Dosage/Direction for Use
Treatment of acute attacks Adult (including elderly) & adolescents >12 yr 1 dose vial every 6-8 hr. Childn 2-12 yr 3 drops/kg/dose (max: 2,500 mcg of salbutamol) every 6-8 hr. Maintenance treatment: 1 dose tid-qid.
Contraindications
Hypersensitivity to atropine or its derivatives. Hypertrophic obstructive cardiomyopathy or tachyarrhythmia.
Special Precautions
Hypersensitivity. Discontinue immediately if paradoxical bronchospasm occurs. Ocular complications (ie, mydriasis, increased IOP, narrow angle glaucoma, eye pain) may occur if soln or mist comes in contact w/ the eyes. Insufficiently controlled DM, recent MI, severe organic heart or vascular disorders, hyperthyroidism, phaeochromocytoma, risk of narrow-angle glaucoma, prostatic hypertrophy or bladder neck obstruction. Patients w/ underlying severe heart disease (eg, ischaemic heart disease, tachyarrhythmia or severe heart failure). Hypokalemia. GI motility disturbance in patients w/ cystic fibrosis. Dyspnoea. Interference w/ diagnostic tests or other diagnostic measures; may lead to +ve result in test for nonclinical substance abuse. May impair ability to drive & use machines. Pregnancy & lactation.
Adverse Reactions
Headache, throat irritation, cough, dry mouth, GI motility disorders (including constipation, diarrhoea & vomiting), nausea, & dizziness.
Drug Interactions
Not recommended w/ other anticholinergic drugs. Increased hypokalaemia w/ xanthine derivatives, glucocorticosteroids & diuretics. Increased susceptibility to arrhythmias w/ digoxin. Enhanced effects w/ MAOIs or TCA. Increased susceptibility to CV effects w/ halogenated hydrocarbon anaesth eg, halothane, trichloroethylene & enflurane.
